Is it OK to choose testosterone suspension for TRT?

No, it’s the least suitable form of Testosterone for TRT. On testosterone replacement therapy, you need your Testosterone to have the longest possible half-life. Suspension has the shortest one. For TRT, go for Apoxar TRT or other forms of Testosterone Decanoate (or, at least, Enanthate). 

Is testosterone suspension painful? 

Yes, suspension is more painful than most oil-based anabolic steroids during injection.
